Isaac (Richard Gere) is a psychiatrist, treating a woman (Uma Thurman) with a recurring violent dream. He talks to the girl's sister Heather (Kim Basinger) and soon embarks on an illicit affair with her. She says she is falling in love with him, and promises to leave her Mafiosi husband. This threat turns deadly when she murders her spouse. Isaac must defend Heather in court by proving that she was temporarily insane, but are her claims of madness just a devious alibi?
